Workshop & Tour Registration Forms
Thank you for registering for a tour or workshop with Adam Schallau Photography. To complete the registration process, please download, complete, and submit a copy of the Registration Form and the Acknowledgement of Risk Form.

Step 1. Download the Forms
You can download both the Registration Form and the Acknowledgement of Risk Form from our Dropbox site at the following link: Download the Workshop & Tour Registration Forms
Step 2. Filling-In and Completing the Forms
Both forms are PDF files and can be opened and filled in with the free Adobe Acrobat Reader. Your computer may already have Adobe Acrobat Reader installed on it. Please be sure to complete all fields on the forms and sign both of them. If you're not sure how to sign your form electronically, you can find instructions for how to sign a PDF on the Adobe website. Lastly, be sure to save both forms.
Step 3. Send Me the Completed Forms
After you have completed and saved the forms, you can send them to us via email. That's all there is to it. Our email address is on the forms (we purposely don't post our email address on the website in order to avoid being bombarded with junk mail from spammers).

How to Use Your Smart Phone as a Scanner
You should not need to print the forms, but if for some reason you do need to print them, you can then use a scanner or smartphone to digitize the forms and save them as a PDF. If you have a smartphone, you can use its camera as a scanner to digitize any documents you have to print out.
iPhone users: you can use the Notes app to scan a printed document and convert it to a PDF. You can find easy-to-follow directions on Apple's website for scanning documents in Notes using the camera on the iPhone.
Samsung Galaxy users: you can find instructions on the Samsung website for scanning documents with your Samsung phone's camera app.
Tips for Scanning Documents with Your Smartphone
If you need to scan the forms with your smartphone, it is important to do the following:
- Lay the form flat on a table or counter.
- Make sure the form is evenly lit and in bright light.
- Be as still as possible so the resulting photo is sharp.
- Be sure to scan/photograph each page in the forms.
